Certification Program Registration Form
Due to the increasing number of no-show registrants, we require a $50 non-refundable deposit to accompany your registration. Upon receipt of your registration and $50 deposit by credit card using our “Pay Now” button, you will receive a confirmation email followed by an invoice for the $3,850 tuition and a W9 for processing by your Accounting Department.
Please fill out the following form to sign up for one of our certification programs. This form will be submitted to our training coordinator who will contact you to confirm details of the certification program before finalizing your registration.
